1. Promoter

The prize draw is operated by Midland Eye (Eye-Docs Limited), trading as Midland Eye.

2. Eligibility

  • Entry is open to patients aged 18 years or over who have received treatment or attended an appointment at Midland Eye.
  • Employees of Midland Eye and their immediate family members are not eligible to enter.

3. How to Enter

  • To enter the prize draw, participants must:
    • Submit a genuine review or feedback relating to their experience with Midland Eye; and
    • Complete the prize draw entry form.
  • Reviews may be submitted via Google, Trustpilot, Facebook, or other recognised review platforms.
  • Feedback may be positive, neutral, or negative. Entry into the prize draw is not dependent on the content or rating of the review.

4. Entry Period

  • Entries received during each calendar month will be entered into that month’s prize draw.
  • Entries received after the closing date will be entered into the following month’s draw.

5. Prize

  • One winner will be selected each month.
  • The prize is one £50 voucher.
  • The voucher type may vary and will be determined by Midland Eye.
  • The prize is non-transferable and no cash alternative will be offered.

6. Winner Selection

  • One winner will be selected at random from all eligible entries received during the applicable entry period.
  • The winner will be contacted using the details provided on the entry form.
  • If a winner cannot be contacted within 14 days, Midland Eye reserves the right to select an alternative winner.

7. Verification

  • Midland Eye reserves the right to verify entries and request additional information where necessary.
  • Entries that cannot be reasonably verified may be excluded from the draw.

8. Data Protection

  • Personal information submitted as part of the prize draw will be used solely for administering the prize draw and contacting the winner.
  • Data will be processed in accordance with Midland Eye’s Privacy Policy.

9. Publicity

  • Midland Eye may request permission to announce the winner’s first name and town/city. Consent will be obtained before any such publication.

10. General

  • Midland Eye reserves the right to amend, suspend, or withdraw the prize draw at any time where necessary.
  • By entering, participants agree to be bound by these Terms & Conditions.

Clinical results can vary depending on individual patient factors, medical history, and response to treatment. While we strive to provide the highest standard of care, all medical procedures carry potential risks and uncertainties.
